At Dr. Todd Roby, DDS we run our office differently than other dental practices. Your initial appointment will take 60-90 minutes and will consist of a comprehensive examination, a TMD (jaw joint) examination, digital photos of your teeth, dental x-rays and a review of your treatment options. However, a complex medical history or treatment plan may require a consultation once all of your diagnostic information has been reviewed. Dr. Roby wants to get to know you and offer you complete and personalized dental care. We are not a “one size fits all” type of practice!
Once your treatment plan has been determined we will schedule the appropriate type of cleaning appointment for you and any necessary treatment visits.
Please assist us by providing the following information at the time of your initial examination:
Any x-rays taken by a previous dentist, if current within the last six months.
A list of medications you are presently taking.
If you have dental insurance, bring the necessary completed forms or your id card. This will save time and allow us to assist you in the processing of any claims.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, knee or hip joint replacements, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.).
If you are a new patient or you need to update your medical history, please download the patient forms below. Once it is down loaded, you may type in the information and then print it. Please bring the completed printed form with you when you come for your appointment.
Insurance & Payment Options
We accept cash, check, Visa, and Mastercard as payment for your balance. In addition, we offer a 5% discount for cash and local check. We deliver the finest care at the most reasonable cost to our patients, therefore payment is due at the time service is rendered unless other arrangements have been made in advance. We accept all dental insurances and are a participating provider of Delta Premier, United Healthcare, and Cigna Discount Plan.
Please remember you are fully responsible for all fees charged by this office regardless of your insurance coverage.
Most insurance companies will respond within four to six weeks, any remaining balance after that time frame becomes your responsibility. Your prompt remittance is appreciated.
If you are interested in a Payment Plan, one is available for our patients through Care Credit. You can pick up an application form in our Longmont office, or you can go on line and fill out an application before you come to the office.
